Additional Charges to the Complaint
Over the years I have complained about the performance of Kaiser Per-
manente employees. I am a Kaiser member number 14940027.
These are notes which are subject to revision or modification.
I met with Dr. Barsanti, who I believe was a neurologist. He refused to
honor my wishes that I did not want to do a biopsy for the prostate area
until I was able to have time to think about it. At some point I was either
switched or requested another doctor named Patel.
I also switched from my primary doctor to Dr. Jeffrey Siegel. Upon being
assigned to Dr. Siegel since my medical record was about 2000 pages at
the time, I asked Dr. Siegel to provide me with a letter requesting a rea-
sonable accommodation to my employer and also to my housing
provider. The doctor Siegel refused, and his nurse said that the doctor
wanted to see me for more visits before he could consider writing such
letters. I feel that since my medical record was 2000 pages long, that Dr.
Siegel was not justified and saying that he could not write the letters yet.
I felt that the real reason for his decision was discrimination against me
as a black male because of my race. I raised the issue of discrimination in a message to the
Kaiser portal on November 1, 2025 to Jessica and I quoted the Kaiser code of conduct re-
garding discrimination harassment and disrespectful behavior. I also at-
tached a letter to Kaiser dated October 31, 2025.
I wrote Feb 3, 2026 to kaiser.org portal.
Against Dr. Thipp
Who can provide a verification letter? Someone who knows about the
needs of the person who is requesting the reasonable accommodation or
modification can provide a request for reasonable accommodation
verification letter. According to DFEH and HUD, a medical verification
letter request for reasonable accommodation should also state [The
requested accommodation] is necessary for [patient/client name] to live
in the community and use and enjoy [his/her/their] dwelling by
[describe how the accommodation will assist or support the individual].
Source Google. Dr. Christopher Thipphaving provided me a letter
December 23, 2025 but it does not state why the accommodation is
neccessary and in accordance with my ailments described by him.
The doctor is unresponsive to my concerns in my Jan. 26, 2027 letter to him
thru the Kaiser portal. I reference his December 16 , 2025 portal letter to
me at 2:10 pm which I believe takes an incorrect position.

2025 portal letter to me at 7:03 pm which I believe takes a biased
position and shows an ignorance of the many letters I wrote to the portal
detailing my housing needs.
I was to write the medical records department around 2025 by email and
requested that the letters for reasonable accommodation be written by
three doctors, including Siegel, Kazimiroff, and the psychiatrist Thipp, in
which they would have to verify my ailments. But the medical records
department advised me by email that all doctors refused to participate,
and provide those letters. I feel this action occurred because of
discrimination because of my race, Black.
Dr. Thippenvong did write a letter eventually, but it was inadequate. Dr.
Boris Kazimiroff certified my need for a handicap placard, but no reason
was given why he would not provide the reasonable accommodation
letter although there was extensive communication through the portal
with Dr. Kazimiroff but I feel that the end result was that he did not
write the letters because of my race, Black.
Around December 2024 was when I first requested letters of
accommodation from the doctor. Around April 2026, Dr. Jeffrey Siegel
would eventually write a letter requesting a reasonable housing
accommodation. The letter was dated April 2026 but not mailed to me
until June 2026. My question with the letter is that he only identified
one of my numerous ailments. No reason was given why only one
ailment was mentioned out of over 3000 page medical record. I feel that
only one ailment/symptom was mentioned simply because of racism
against me as a Black male. Sixteen months passed of me begging and
begging.
I asked Kaiser to investigate for discrimination and I filed a complaint
with their grievance and appeals operations. They responded in June 12,
2026 stating that they did not feel there was sufficient information to
confirm any intentional discrimination or unequal treatment by Kaiser
Permanente at this time. I know here that this was not in anyway a
proper investigation because no one interviewed me over the phone or in
writing in reference to the complaint and no one sought to get my input
further after the grievance is filed, nor did Kaiser engage in the
McDonnell Douglas balancing act. I feel that this denial of my complaint
is evidence of the discrimination and establishes the causal connection
between the discrimination that has occurred to me.
In May 27, 2026 communication from the Kaiser portal from Adriana
Duarte RN, she states that the Dr. Siegel the primary has placed a
referral to the urology department at Kaiser Sunset for a follow up care.
There is no explanation why I’m being referred to Sunset, which is a
further away location, and a hardship foe me to get to being that the
doctors know I use a wheelchair. I have asked Adriana in writing to
explain why I would be referred to Sunset and I’ve asked for a reasonable
accommodation that I’d be allowed to be seen at the Cadillac location.
No one at Kaiser has responded to that May 27 request. I feel that the
failure to respond is a denial of my request for accommodation and
discrimination against me because of my disability.
I was told at one point that the Kaiser doctors felt that I had attacked
them. When I asked for proof of any complaints, none was provided to
me. I feel that Kaiser did not have a legitimate reason for saying that I
was attacking them. I feel that the reason why they said I was attacking
them was because of discrimination due to my race, Black, and due to
the fact, I complained about discrimination and inferior medical
treatment.
I have established the causal connection between the actions that
occurred to me and the discrimination statute. I feel that the causation
has been proven, and that discrimination is the proximate reason for the
conduct directed at me.
I also questioned numerous doctors through the kaiser.org portal as to
the timeframe for keeping a catheter in a person’s body. Not one doctor
responded to me in writing. In addition, the current urologist did not
respond, he did not give me adequate reason for me being prescribed
Flomax and at one point, he prescribed an antibiotic for me, and yet no
urine culture had been taken, even though this was the time Kaiser was
on strike, I feel that this urine culture still could’ve been done and still
when I complained, it wasn’t done. My wishes at Kaiser are not being
respected as approximate cause of the discrimination that I have
suffered. Around April 2026 my communications to the kaiser.org portal
were restricted so that I would no longer be able to compose messages or
reply to messages in the portal. I feel this occurred because the doctor
threatened that they would do this to me if I continue to ask questions,
as I am a Black American and Kaiser took the inferior actions against
me. Kaiser had actual and constructive knowledge of my race being
Black.

In 2008, to the Kaiser Portal, I noted, “Occasional vertigo like symptoms body poles to left. Notice sometimes I hold my breath. I do not pass out but feels like I will fall over period i have read this couldn’t relate to magnesium deficiency. Loss of hearing right ear. Occasional hot flashes.”
In 2016, to Kaiser portal Dr. Ethnasios, I write, I continue to suffer from that unpredictable jerking sensation. Feels like balloon swelling inside period the jerking always to the left never happens twice in a row comma almost never while standing, never while driving, almost never while walking. I did get a recent plaque test from a wellness center where they put that device on your neckI think they said I had 50% plaque in the arteries.
Around December 2023, I am back under the care of Kaiser. I give them a letter from a non-kaiser Dr, Marcus Tellez, who says in April of 2023 that I have a medical condition “that limits his major life activities. As a result of his disability, he occasionally experiences episodes of disorientation and dizziness, balance issues, and trouble maintaining bodily fluids.” The doctor details various accommodations and the reasons for the request. The Tellez letter is shared with Kaiser doctors around January 2, 2024 to Doctor Patel. This is my first notice to Patel that I need a similar letter to be generated by Kaiser and my notice of what I want the letter from Kaiser to say.
On March 12, 2024 I wrote to the Kaiser portal “2024-3-12 Letter Notes to Kaiser.pdf” . I remind the doctors of the January 11, 2024 letter and and “I ask that you support this letter as it states my medical conditions.” I note: “There has been no response from Kaiser on this issue.” This is sent to Kaiser doctors Patel, Dr. Rudell, Dr. Rivaz. See attached receipt.
On March 25, 2024 to Kaiser doctors I write “Part III Medical Observations” indicating medical ailments normal routine, walking, driving, and the trigger.
On July 4, 2024 I request a (written) differential diagnosis from Dr. Rivaz. See attached.None is provided.
On August 1, 2024, I write doctor Patel requesting a RA for my employer as well as housing provider. By September 2024, The Patel duties will soon be taken over by Doctor Siegel when I switch doctors. I indicate that the letters I request are similar to the already provided Tellez letter. I make suggestions for the contents of the letters. See attached “final for medical notes” 2024-8-1.
On November 4, 2024, I met with Dr. Rivaz. In my review of the appointment, I reiterated my request for the letters of RA. See attachment.
As of December 2024 and forward, I have not received any information that Kaiser will provide me with the requested letters for reasonable accommodation or any related forms.
March 7, 2025 I communicated to Dr. Rivaz after an appointment. See attached. She did not mention in the appointment that Kaiser has provided the requested letters requesting RA.
